I hereby consent to the use of my name(s), photographs or other likenesses of me in any present or future information materials, internet sites, broadcasts, telecasts or other promotional forums which the Miniature Golf U.S. Open Tournament Committee, the U.S. Prominigolf Association, Hawaiian Rumble, Hawaiian Caverns, Sutters Mill, the TEAM USA Promingolf Training Center, or the USPMGA Championship Tour may develop, which may reveal my participation in the Miniature Golf U.S. Open Tournament or other related event, without further compensation to me other than the opportunity to win and earn the cash purses and prizes set forth in this and other tournaments.
